CO2点阵激光联合调Q 1 064nm激光治疗创伤性文身伴增生性瘢痕疗效观察

    [摘要]目的:探討CO2点阵激光联合调Q1 064nm激光治疗创伤性文身伴增生性瘢痕的疗效。方法:选取2017年6月-2019年6月在笔者医院就诊的创伤性文身伴增生性瘢痕患者52例,根据随机数字法将患者分为对照组和观察组,每组26例。对照组采用CO2点阵激光进行治疗,观察组采用CO2点阵激光联合调Q1 064nm激光治疗。比较两组患者的疗效,采用改良温哥华瘢痕量表(VSS)对患者增生性瘢痕进行综合性评估,并记录患者水肿、红斑、色素沉着、感染等不良反应情况。结果:治疗后,观察组治疗总有效率92.31%,高于对照组的73.08%,两组比较有显著性差异(P<0.05);观察组VSS评分低于对照组(P<0.05)。观察组不良反应总发生率19.23%,低于对照组的46.15%(P<0.05)。结论:CO2点阵激光联合调Q1 064nm激光治疗创伤性文身伴增生性瘢痕可显著提高疗效,减少不良反应的发生,值得临床推广应用。

    Curative Effect of CO2 Fractional Laser Combined with Q-switched 1 064nm Laser in the Treatment of Traumatic Tattoo with Hypertrophic Scar

    Abstract:Objective? To investigate the curative effect of CO2 fractional laser combined with Q-switched 1 064nm laser in the treatment of traumatic tattoo with hypertrophic scar. Methods? 52 patients with traumatic tattoos and hypertrophic scars had been treated in our hospital from June 2017 to June 2019. According to the random number method, the patients were divided into two groups: the control group (n=26) and the observation group (n=26). The patients in the control group were treated with CO2 fractional laser, in the observation group were treated with CO2 fractional laser combined with Q-switched 1 064nm laser. The curative effects of the two groups were compared, the modified vancouver scar scale (VSS) was used to comprehensively evaluate the hypertrophic scar of the patients, and the adverse reactions such as edema, erythema, pigmentation and infection were recorded. Results? The total effective rate of treatment in the observation group (92.31%) was higher than that in the control group (73.08%), and the difference was statistically significant (P<0.05). After treatment, the VSS score of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). The total incidence of adverse reactions in the observation group (19.23%) was lower than that in the control group (46.15%), and the difference was statistically significant (P<0.05). Conclusion? The CO2 fractional laser combined with Q-switched 1 064nm laser in the treatment of traumatic tattoo with hypertrophic scar can significantly improve the curative effect and reduce the occurrence of adverse reactions, which worthes of clinical application.

    CO2点阵激光利用光热反应原理,使激光作用于皮肤后产生大量柱状热损伤带,局部形成组织凝固带,周围正常组织迁移,从而达到修复目的[17-18]。笔者通过长期的激光治疗操作,认为CO2点阵激光具有穿透力强、安全性高等特点,可汽化皮肤浅层组织,在创伤性文身的修复中起到了一定作用。此外,调Q1 064nm激光易被血红蛋白吸收转化成热量,造成血管封闭,纤维细胞及血管内皮细胞死亡,降解胶原蛋白,从而治疗增生性瘢痕[19]。同时,调Q1 064nm激光可有效穿透真皮组织,使皮肤色素吸收光能,破坏色素,进而清除创伤性文身。其疗效确切、创伤小,易被患者接受[20]。

    本研究中,观察组治疗总有效率高于对照组,观察组VSS评分及不良反应总发生率低于对照组。结果表明,相较于单一使用CO2点阵激光,CO2点阵激光联合调Q1 064nm激光治疗创伤性文身伴增生性瘢痕可顯著提高疗效,减少不良反应的发生。笔者通过反复地操作演练,熟记实际操作步骤,术后相互学习,总结治疗经验,认为可能原因是:CO2点阵激光能量大、创伤小,可确保局部皮肤组织的完整,但缺乏良好的选择性,且易对治疗区周围正常组织造成热损伤,增加瘢痕形成的风险[21-22]。而调Q1 064nm激光是根据光热分离原理,使激光的热效应最大限度地限制在靶组织内,对周边正常组织的损伤较小,从而有效治疗创伤性文身及增生性瘢痕[23]。此外,针对于CO2点阵激光联合调Q1 064nm激光治疗,临床上多适用于颗粒沉积及瘢痕较浅的患者,具有一定的局限性。而严重创伤性文身伴深层瘢痕的患者需采用手术方法进行修复治疗[24]。临床上应根据患者自身情况选择适当的修复手段,并进行个体化治疗,从而提高患者生活质量。

    综上所述,CO2点阵激光联合调Q1 064nm激光治疗创伤性文身伴增生性瘢痕可显著提高疗效,减少不良反应的发生,值得临床推广应用。
