皮肤再生医疗技术对糖尿病大鼠创面愈合及血管生成的影响

    【摘要】?目的?觀察皮肤再生医疗技术(moist exposed burn therapy/moist exposed burn ointment,MEBT/MEBO)对糖尿病溃疡创面愈合及血管生成的影响,探讨MEBT/MEBO促进糖尿病创面愈合的机制。方法?SPF级Wistar雄性大鼠24只,STZ注射建立糖尿病大鼠模型后随机分为对照组及MEBO组,各12只,背部切除建立创面,对照组创面给予生理盐水外敷治疗,MEBO组创面给予湿润烧伤膏(moist exposed burn ointment,MEBO)外敷治疗。于治疗第5天、第11天观察创面愈合情况及病理变化,行微血管计数,并采用免疫荧光法检测创面组织CD31水平。结果?第5天,两组创面愈合率比较差异无统计学意义(P>0.05),第11天,MEBO组创面愈合率高于对照组(P0.05),第11天,MEBO组创面病理恢复情况优于对照组,微血管计数高于对照组(P0.05),第11天MEBO组明显高于对照组(P<0.001)。结论?MEBT/MEBO可通过促进糖尿病创面新生血管形成而促进创面愈合。

    【Abstract】?Objective?To observe the effect of moist exposed burn therapy/moist exposed burn ointment (MEBT/MEBO) on wound healing and angiogenesis of diabetic ulcers, and to explore the mechanism of MEBT/MEBO in promoting diabetic wound healing.Methods?After establishing diabetic rat model by STZ injection, 24 male Wistar rats of SPF grade were randomly divided into control group and MEBO group, with 12 rats in each group. The wounds in the control group were treated with saline by external application, and the wounds in the MEBO group were treated with MEBO by external application. And then, the wound healing and pathological changes were observed on the 5th and 11th day after treatment, microvessel count was performed, and CD31 level in wound tissue was detected by immunofluorescence method.Results?On the 5th day, there was no statistically significant difference in wound healing rates between the two groups (P > 0.05), on the 11th day, the wound healing rate of the MEBO group was higher than that of the control group (P 0.05), on the 11th day, the pathological recovery of the MEBO group was better than that of the control group, and the microvessel count was higher than that of the control group (P 0.05), but on the 11th day, the MEBO group was significantly higher than the control group (P < 0.001).Conclusion?MEBT/MEBO can promote wound healing by promoting neovascularization of diabetic wound.

    糖尿病创面是糖尿病常见的严重并发症之一,是一种慢性难愈合创面[1]。由于发病原因复杂,机制尚未完全明确,临床治疗很困难。中医药在慢性处理中具有独特效果,皮肤再生医疗技术(moist exposed burn therapy/moist exposed burn ointment,MEBT/MEBO)在临床上应用30余年,对糖尿病等慢性创面疗效显著[2~4],但具体机制尚未阐明。新生微血管形成在创面愈合中具有重要作用,本研究拟通过观察MEBT/MEBO对糖尿病创面及创面血管生成的作用,探讨MEBT/MEBO促进糖尿病创面愈合的机制。

    1?材料与方法

    1.1?实验动物

    SPF级Wistar雄性大鼠24只,3月龄,体重230 g~260 g,由长沙市天勤生物技术有限公司提供。该实验经右江民族医学院动物伦理委员会批准。

    1.2?方法

    1.2.1?糖尿病创面模型的建立

    大鼠适应性喂养1周,禁食16小时,腹腔注射STZ建立糖尿病大鼠模型,1周后大鼠出现糖尿病症状且空腹血糖≥16.7 mmol/L为模型建立成功。在糖尿病模型组大鼠成功建立1周后,腹腔注射水合氯醛进行麻醉,备皮,碘伏消毒,在大鼠背部行直径约为2.5 cm×2.5 cm的全层皮肤切除创面,创面深度要求达到筋膜层,建立糖尿病创面模型。

    1.2.2?实验分组及药物干预

    大鼠随机分为对照组(糖尿病模型组)和湿润烧伤膏(moist exposed burn ointment,MEBO)组,各12只,创面每日换药2次,对照组给予生理盐水纱布外敷,MEBO组给予湿润烧伤膏(汕头市美宝制药有限公司生产,国药准字Z20000004,40 g/支)纱布外敷,每日检测血糖并予胰岛素常规控制。

    1.2.3?标本采集与处理

    分别于第5天、第11天2个时相点麻醉后采集创面组织(第11天为公认的糖尿病创面修复期)[5]。各组织样本均给予10%福尔马林固定制作HE染色切片和CD31免疫组化检测。

    1.2.4?创面平均愈合率

    分别于上述2个时相点,以刻度尺作为参照,并对创面进行拍照,采用Image J软件测量未愈合创面的面积并计算愈合率,创面愈合率=(原始的创面面积-未愈合的创面面积)/原始的创面面积×100%。

    1.2.5?HE染色观察病理改变并进行微血管计数

    将采集的创面组织放置于10%福尔马林溶液中固定24小时后,行石蜡包埋后制作病理切片,使用光学显微镜观察创面形态学变化并行微血管计数(每张切片由3位观察者分别计数,取平均值)。

    1.2.6?免疫荧光CD31染色

    组织切片脱蜡水化后按流程进行CD31免疫荧光染色,观察血管标记物CD31表达差异。

    1.3?统计学方法

    采用SPSS 17.0统计软件处理数据,计量资料符合正态分布,以(±s)表示,组内第5天、第11天比较采用配对t检验,组间比较采用两独立样本t检验,检验水准:α=0.05,双侧检验。

    2?结?果

    2.1?創面愈合率比较

    第5天,两组创面愈合率比较差异无统计学意义(P>0.05),第11天,MEBO组创面愈合率高于对照组,差异有统计学意义(P<0.001)。见表1。

    2.2?创面病理组织及微血管数量比较

    第5天,两组创面均表现为组织水肿、大量炎症细胞浸润、少量纤维细胞及毛细血管形成,其中对照组水肿及炎症细胞浸润较MEBO组更严重;第11天,两组创面炎症细胞浸润减轻,肉芽组织形成,对照组炎症细胞浸润更明显,成纤维细胞及毛细血管分布较MEBO组稀疏紊乱。微血管形成数量比较显示:第5天,两组比较差异无统计学意义(P>0.05),第11天,MEBO组数量多于对照组(P<0.001)。见图1,表2。

    2.3?免疫荧光CD31染色比较

    CD31免疫荧光染色显示:第5天,两组OD值比较差异无统计学意义(P>0.05),第11天,MEBO组表达明显多于对照组(P<0.001)。见图2,表3。

    3?讨?论

    糖尿病创面是常见的慢性创面,慢性创面愈合是目前医学难题,涉及新生血管生成、细胞生长、外基质形成等,其中新生血管形成是创面愈合的关键,在创面愈合过程中具有十分重要的意义[6~7]。新生微血管形成后,可给创面细胞提供养分,促使创面细胞生长,肉芽组织形成,促进创面愈合。新生血管越多,能为创面提供更多的氧气和营养成分,肉芽组织生长越快,创面越容易愈合[8~9]。

    中医药治疗糖尿病足历史悠久,MEBT/MEBO基于“去腐生肌、煨脓长肉、活血化瘀”等机理促进创面愈合,已有研究表明其能提高溃疡创面肉芽组织血管内皮生长因子(vascular endothelial growth factor,VEGF)水平,促进创面生长[10~11]。本研究表明,采用MEBO处理创面后,至第11天,创面愈合率明显高于仅采用盐水干预的糖尿病模型组,表明了MEBO对糖尿病创面愈合具有良好的促进作用。进一步研究显示,MEBO组新生血管计数及血管阳性标记物CD31明显高于对照组,表明MEBO可有效促进糖尿病创面新生血管形成,具有促进新生微血管生成的作用,通过促进新生血管生成,为创面组织提供养分,促进创面组织的生长,创面愈合。

    MEBO是纯中药成分,主要组分有黄柏、黄连、地龙、黄芩、罂粟壳等,通过现代医学手段,发现其主要含β-谷甾醇、黄芩甙、小檗碱等,临床实践已证实这些药物成分具有活血化瘀、祛腐生肌、煨脓长肉等功效,基于这些功效,从中医理论看,MEBO兼具促进组织细胞生长血液循环、新生血管再生等作用。多项研究表明,MEBO可通过调控PI3K/AKT信号通路调控VEGF分泌,从而促进内皮细胞生长,新生血管形成,本实验结果也显示,经MEBO干预治疗后,创面血管阳性标记物CD31升高,新生血管数量增多,验证了MEBO的促血管生成作用。综上,MEBT/MEBO可有效促进糖尿病创面愈合,促进新生血管形成是其重要机制之一。
